Plaque Order Form

From February 1 to June 1, directors may order plaques for choirs receiving Excellent ratings at District and State Choral Festivals. Paid orders of $50 per plaque will be placed by the State Office and shipped to the address indicated on the Plaque Order Form.

Paid orders will be placed on April 15 and June 30.  

Please contact the State Office if you need to order plaques for years prior to 2024.
